A card has to stand alone. If it leans on context you'd have to go dig up, or on an acronym nobody defined, it doesn't reach you.
Speak a reaction and it lands as a reply in the thread. Typing a paragraph on a phone is where good judgment goes to die.
The agent that owns the work picks up your reply in the same thread, so a decision turns into action while you're still holding the phone.
Most approval tools ask “are you sure?” about everything, which trains everyone to tap yes without reading. The Desk sorts by one question: can this be taken back? If it can, your tap acts at once and you get a few seconds to change your mind. If it can't, because a message reaches someone outside or money moves or something leaves the building, you confirm on purpose before anything fires. The weightier the call, the more deliberate the confirm.
An undo window is an approval gate run backwards.
Each decision is written down with who made it, when and what they chose. It survives a reload and a phone swap. Ask an outcome who approved it and you get a name, not a shrug.
